Call Forward External

I have an Option 61C Rls. 24 software with 3 two-way PRI's.

I have a phantom set to external call forward.

first call from an external number to phantom routes out ok, second call get error message from CO.

Internally it will forward more than one at a time with out any problem. Could this be a problem with a setting on the DCH? or some other programing problem on the PBX or am I looking at a telco problem?
