Question NTRB21 card install

Sorry about what is sure to be a dumb newbie question:

We are running an Opt11 C Rel 25 in a lab used for our telecommunications technology classes (so this is a non-production system). Up to now we were mainly using digital lines and analog trunks to give students hands-on experience with standard MAC operations.

We are now trying to add an NTRB21 T1/PRI card (with clock controller daughter card). After initialization, the card has both the OOS and the ACT lights on, which is a bit odd, and the CC card light also indicates disabled.

My main problem is that the card seems to be invisible to the switch; a STAT command returns CARD UNEQ, and all other attempts to access the card result in similar errors. The physical T1 is not yet connected, so I expect a red alarm, but should the card not at least show up in the switch?

I am sure we skipped a terribly basic initialization command, but can someone point me in the right direction?
