Question VOIP Packet Loss IPO

We run an IPO 412 (3.0) with a VCM 20 and 4610SW phones , Sonicwall TZ 170 (office and home offices with the VOIP software installed), and all appropriate QOS hardware and CAT 6 cables. Overall, everything works great with great QOS, however, a few of our VOIP extensions experience packet loss during the upload phase (VOIP side speaking) and the listener hears garbage. From a download experience (VOIP extension listening), quality is excellent. All VOIP extensions are on business quality cable broadband and seem to be averaging between 200-300 kbps while on the phone (up and down) when checked with DSL Reports and simliar services. Our corp office is on a PRI and averages about 380 kbps up and down consistently and is rated at 512kbps. Does anyone have any ideas on what to check, and how, to trace down the culprits for packet loss for our VOIP extensions? What is the minimum speeds we should see while on the VOIP phones? Other than basic e-mail, our Internet is not heavily used. I appreciate any and all feedback.

If you have use AVAYA AIM than check for loss in VOIPMonitoring (VMON)tool. Check real time packetloss and also historical data.
