How do I set up out going Caller ID?

How do you set up the outgoing name an number for a PRI on the definity?

You would need an ISDN PRI to support CallerID.

in the PBX type: change isdn public-unknown-numbering

Ext Len - 4 (for 4 digit dialing in your dial plan)
Ext Code - 1 (for 1xxx extension)
CPN Prefix - 801555 (prefex to the extension)
Total CPN Len - 10 (ie 8015551000 - 10 digits)

when extension 1000 dials out he will have callerid of 8015551000

What happens when you have multiple exchanges in the same system using 3 digit dialing?

Very Easy!
Just change the Ext Len to 3. and keep the CPN Length at 10.
then lets say 1xx and 2xx go to prefix 801-444-4xxx
and lets say 3xx and 4xx go to prefix 801-555-5xxx then:

Ext Code CPN Prefix
1 8014444
2 8014444
3 8015555
4 8015555
Extension 144 will show callerID of 801-444-4144

Another example if your extension is 234 you could do this:

234 9005551212
That extension would show callerID of 900-555-1212

how do you set it up so that everyone's calls from the PBX will show the main number only?

I would do it like this:

Ext Code CPN Prefix
1 8015551212
2 8015551212
3 8015551212
4 8015551212
5 8015551212
6 8015551212
7 8015551212
8 8015551212

that will force all extensions to display 8015551212 as their callerID
