441T is a CO message number. You are getting to the CO but stopping there.

User was able to dial 9054726021 and got thru succesfully twice....but in the third attempt the call failed and got same error message.

I've had a problem with our current ld provider twice in the last six months. Each time the problem was on their end and not mine. Both were for overseas calls and the second time they refused to fix it. Needless to say, we are shopping for a new ld provider. The first time I racked my brain and couldn't find anything wrong, when I forced the call out of a different trunk, then the number the call would go through fine. I guess what I am saying is sometimes the provider is at fault and it is a good idea to not always think that the problem is on your end...

does that mean this is a Telco CO switch problem and not a problem with our pbx?

I am inclined to say it is because you are getting the CO error message plus your trace shows the complete number hitting the trunk. When you place the call from a cell phone or from another location the call completes. But when you do that you are using a different provider. This will be difficult to prove but the fact that you were able to make the call successfully a couple of times tells me it is not in your PBX. They have a CO problem of some kind, in my opinion.
