The real cost is the Broadcast licence required to play any music on hold music that is licenced - you must seek the rights of the artist and pay the royalty broadcast fee to use it. There are certain alternatives such as music >100 years old or something like that: i.e. classical music.

To get decent quality you need something like the Linemaster -> a simple line
matching and amplifying unit designed to improve the sound
quality and volume from a standard audio source such as CD
players when connected to the MOH port of a telephone
