Your DHCP scope needs to be setup for the phones. It would appear you are missing the Option 176 info in the scope which is where the Call Server and File server info are stored.
"A good start would be Appendix D in the IP Telephony Deployment Guide which explains the DHCP setup and options. It can be found on the Avaya Support Web (or the 4600 Series IP Telephone - LAN Administrator Guide - Section DHCP).
Another good reference for the boot-up sequence for the phones can be found in the Avaya IP Implementation Guide (page 48)"
