If you decide to use EC500 over call forward, I would really recommend that you find the EC500 guide in the CM Docs for your release. While being wordy, it does thoroughly go over all the possible configurations for your site at your release. EC500 at CM3 is so much easier than those Xmoble tables.
But the other boardmembers(heh) are right, the user is forwarding the call, not using EC500. If you want to find out what it's no longer working, I would recommend tracing the phone both when the button is pressed, and when placing a call to it when enabled.
