Al, I was not able to open the PDF you referenced, perhaps it is a restriction here at work.
I suspect it is a simple problem. When the connect window opens on your computer, click on properties and then go to the security tab. Click on "Show terminal window" then click on Ok. Back at the connect window, clear the User Name field and do not click on the save button (that will insert data that will cause your connection to fail).
This time when you dial a black window will open and this is where you enter the remote login and password.
