What is the vintage and release of the board? Have you updated the firmware?
Also (and this is important) do you have the black adapter on the back of the switch on the MedPro slot, the gray one does not support 100M FD. And is the cabling from the amphenol the adapter plugs into, to the back of the carrier cat5?
You can tell by looking at the six inch long jumpers, if they are all white with a couple of red leads it is cat3. It will not run stable at 100M FD. If it is multicolored (like a 25 pair cable) then it is cat5. Avaya does sell a cable kit to upgrade individual slots to cat5.
